Step 1 - after getting to the OCBOE website - click on the Parents tab.
Step 2 - under parents - select Coordinated School Health
Step 3 - Notice the links on the right-hand side - select from these to find additional resources. From these you can find things such as immunization requirements, Individualized health plans, mental health resources, physical evaluation forms, as well as concussion forms.
